Job summary

Dublin SNF Healthcare, LLC in Dublin, CA is seeking a PRN Physical Therapist to assess residents, develop treatment plans, and provide skilled PT services in a post-acute care setting. Qualifications

• Bachelor's degree in Physical Therapy; advanced degrees preferred.
• Proficient with Net Health and Point Click Care data entry.
• Ability to read and understand policy and procedure manuals.
• Ability to present information clearly to managers, staff and families.
• Strong mathematical skills for fractions, percentages, ratios.
• Able to interpret varied written, oral instructions.
• Effective communication with interdisciplinary team.

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans.
• Supervise PT assistants and staff.
• Communicate progress with physicians, nursing, residents and families.
• Participate in care conferences and rehab meetings.
• Provide in-services and staff training as needed.
• Maintain compliant documentation.
• Ensure infection control and safety protocols.
• Educate residents and caregivers on home exercise programs.
• Recommend durable medical equipment as appropriate.
• Maintain confidentiality.
