Job summary

Greenridge Post-Acute and Senior Living is seeking a licensed Physical Therapist (PT) on a PRN basis. You will conduct comprehensive evaluations, develop and supervise individualized plans of care, and deliver skilled interventions to residents in a post-acute setting in California.

This role emphasizes clinical autonomy, leadership within the rehab team, and direct impact on resident function and quality of life. California licensure and NPTE completion are required, with CPR/BLS preferred.

Qualifications

• Active California Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• NPTE exam completed or eligible for licensure in CA.
• Current CPR/BLS certification (or willingness to obtain).
• Experience in skilled nursing or post‑acute care preferred.

Responsibilities

• Perform comprehensive evaluations and reassessments per physician orders and CA standards.
• Develop and modify individualized plans of care with measurable goals.
• Deliver skilled therapeutic interventions (individual and group).
• Supervise PTAs and rehabilitation staff per CA law and standards.
• Communicate findings and discharge outcomes to physicians, team, patients, and families.
• Instruct patients/caregivers in home exercise programs and use of devices.
