Job summary

The role includes evaluating, planning, and delivering individualized treatment programs, documenting progress, and collaborating with the interdisciplinary team to ensure high-quality care.

Qualifications

• SNF or long-term care experience preferred.
• Knowledge of current therapy practices.
• Experience with Net Health or similar EMR.
• Ability to work with diverse patient populations.

Responsibilities

• Evaluate and treat residents with temporary or long-term disabilities.
• Develop and implement individualized plans of care.
• Deliver treatment to improve mobility, reduce pain, and restore function.
• Educate patients, families, and caregivers on therapy plans and progress.
• Monitor patient responses and adjust treatments as needed.
• Accurately document treatments and patient progress in compliance with regulations.
