• SNF or long-term care experience preferred.
• Strong knowledge of current therapy practices.
• Experience with Net Health (or similar EMR) preferred.
• Ability to work effectively with diverse patient populations.

Responsibilities

• Evaluate and treat residents with temporary or long-term disabilities.
• Develop and implement individualized plans of care.
• Deliver treatment to improve mobility, reduce pain, and restore function.
• Educate patients, families, and caregivers on therapy plans and progress; monitor patient responses and adjust treatments as needed.
• Accurately document treatments and patient progress in compliance with regulations.
