Active In-Home Therapy is seeking a Home Health Occupational Therapist to join our team on a PRN basis in Salinas, California. You will assess patients in their homes, develop customized treatment plans, and provide hands-on OT interventions to promote independence.

Qualifications

• Current state licensure as an Occupational Therapist.
• Experience in home health or related setting preferred.
• Strong clinical and critical thinking abilities.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct patient assessments to determine rehabilitation needs.
• Develop and implement individualized OT treatment plans with the healthcare team.
• Provide hands-on OT interventions for ADLs, fine motor skills, and adaptive techniques.
• Educate patients and families on adaptive strategies and devices to promote independence.
• Monitor and document patient progress and adjust plans as needed.
• Collaborate with physicians, nurses, and physical therapists for comprehensive care.
• Maintain accurate patient records and documentation.
• Communicate effectively with patients, families, and caregivers in a supportive environment.
