Job summary

Sobe Innovative Rehab seeks a licensed Physical Therapist to provide therapy services for Hospice Patients in a home setting in Largo, FL. This PRN role offers flexible assignments and competitive compensation while supporting independence and quality of life for older adults.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Active Florida Physical Therapist (PT) license
• New graduates and experienced therapists are encouraged to apply

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting
• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR

Education
