Job summary

River Falls Post Acute in Marietta, SC, is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team on a PRN basis. You will evaluate and treat residents, develop individualized treatment plans, and collaborate with the interdisciplinary care team to improve mobility and independence.

The ideal candidate holds a Bachelor's degree in Physical Therapy (Master's/Doctorate preferred), a South Carolina license, and experience in skilled nursing or long-term care.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate degree in Physical Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PointClickCare (PCC) and Casamba preferred.
• Current South Carolina Physical Therapist license in good standing; CE requirements must be met.

Responsibilities

• Evaluate residents within 24 hours of physician referral when possible.
• Develop individualized treatment plans and obtain physician approval as required.
• Provide PT treatments per orders and goals; monitor progress.
• Supervise PTAs, therapy aides, and students per state regs.
• Educate residents and families on therapy goals and discharge plans.
• Complete evaluations, progress notes, and documentation timely.
