Job summary

Continuum Therapy Partners, LLC in Graham, NC is seeking a Physical Therapist to provide flexible PRN coverage. You’ll join a team led by experienced clinicians and help maintain continuity of care while crafting your own daytime schedule.

Requirements include a DPT, current NC PT license, NPTE pass, and about one year of prior PT experience. You’ll evaluate and plan care, document progress, educate patients and families on home programs, with coordination across disciplines.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.
