Impact Health PC is seeking a PRN Palliative Care Nurse Practitioner to provide compassionate, patient-centered care for residents in skilled nursing facilities in Christiana, PA. You will assess symptoms, develop individualized plans, and support goals-of-care discussions with patients, families, and interdisciplinary teams.

This role offers flexible PRN scheduling, autonomy within your scope, and strong clinical support from an experienced team to ensure high-quality care.

Formación

• Active NP license in Pennsylvania.
• Minimum 2 years of post-certification clinical experience preferred.
• Experience in palliative care, hospice, SNFs, hospitalist medicine, ED, or ICU preferred.

Responsabilidades

• Provide compassionate, patient-centered Palliative Care in skilled nursing facilities.
• Assess and manage symptoms; develop individualized treatment plans.
• Support goals-of-care discussions with patients and families; collaborate with care teams.
• Complete timely and accurate clinical documentation; ensure regulatory compliance.
