Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop plan of care.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment to cognitive/ADL disorders per requirements.
• Administer procedures as part of rehab plan (ADL retraining; therapeutic activities; exercises; modalities; devices).
• Assess progress and adjust treatments in the clinical record per requirements.
• Evaluate treatment outcomes and modify services; refer as needed.
• Provide counseling to patients, families, caregivers and other service providers.
• Communicate with supervisor and health team about progress, barriers, and plans.
• Provide discharge summaries per requirements.
