Job summary

La Casa Via Transitional Care Center in Walnut Creek, CA is seeking a licensed Occupational Therapist on a PRN basis. You will evaluate, develop, and manage individualized care plans, providing clinical leadership within a Skilled Nursing rehab team.

Work with OTAs and aides under CA regulations, coordinate discharge planning, and ensure high-quality resident outcomes across ADLs, IADLs, and cognitive rehab in a supportive, community-focused setting.

Qualifications

• Must hold California OT license and NBCOT certification.
• Provide comprehensive OT evaluations and plans of care.
• Supervise OTAs and aides per California law and standards.

Responsibilities

• Conduct evaluations and re-evaluations with physician orders and state practice standards.
• Develop, implement, and modify individualized care plans with measurable goals.
• Deliver interventions addressing ADLs, IADLs, cognition, and upper-extremity rehab.
• Lead discharge planning and coordinate care transitions to home or community.
• Communicate findings to physicians, team, patients, and families.
