Job summary

Senior Care Solutions is seeking compassionate PRN Licensed Practical Nurses (LPNs) to join our Medicare-certified home health team in Florida. You will provide skilled nursing visits to patients in their homes, under RN supervision and physician orders, with a flexible schedule that fits your lifestyle.

The role emphasizes independence, compassionate patient care, and collaboration with physicians, case managers, therapists, and the broader care team.

Qualifications

• Current Florida LPN license in good standing.
• 1+ year of nursing experience.
• Home health experience preferred but not required.
• Current BLS/CPR certification.
• Valid driver’s license and reliable transportation.
• Strong communication, organization, and time management skills.
• Ability to work independently while providing compassionate, patient-centered care.

Responsibilities

• Provide skilled nursing visits in patients' homes according to the plan of care.
• Perform nursing assessments and report changes to the supervising RN.
• Administer medications and treatments as ordered by the physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
• Ensure compliance with Medicare Conditions of Participation and agency policies.
