Job summary

Senior Care Solutions is a growing Medicare-certified home health agency seeking compassionate PRN Licensed Practical Nurses (LPNs) to join our Florida team. You will provide skilled nursing visits in patients' homes under RN supervision and physician orders, with flexibility to fit your schedule.

As an LPN, you will build meaningful relationships with patients and families, receive per-visit compensation, and work with an interdisciplinary team while ensuring high-quality care and Medicare

Qualifications

• Current Florida LPN license in good standing.
• 1+ year of nursing experience preferred.
• Home health experience preferred but not required.
• Current BLS/CPR certification.
• Valid driver's license with reliable transportation and auto insurance.
• Strong communication, organization, and time management skills.
• Ability to work independently while providing compassionate, patient-centered care.

Responsibilities

• Provide skilled nursing visits in patients' homes in accordance with the plan of care.
• Perform nursing assessments and report changes in patient condition to the supervising RN.
• Administer medications and treatments as ordered by the physician.
• Educate patients and caregivers on disease management, medications, and safety.
• Complete accurate and timely documentation in the electronic medical record.
• Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
• Ensure compliance with Medicare Conditions of Participation and agency policies.
