Job summary

Senior Care Solutions is seeking a compassionate PRN Registered Nurse (RN) to join our Medicare-certified home health team in Florida. The RN will provide skilled nursing care to patients in their homes, following physician orders and agency policies.

Strong clinical judgment and timely documentation are essential. The role involves Start of Care, ROC, Recertification, routine visits, and discharges, with travel across the specified coverage area.

Qualifications

• Current, active Florida RN license.
• Minimum of one year nursing experience.
• BLS/CPR certification.
• Valid Florida driver’s license and reliable transportation.
• Experience with WellSky/Kinnser is a plus.

Responsibilities

• Perform SOC, ROC, Recertification, Routine, Evaluation, and Discharge visits.
• Complete comprehensive patient assessments, including OASIS when applicable.
• Develop, implement, and update individualized plans of care.
• Administer medications, treatments, wound care, injections, and other skilled nursing services.
• Educate patients and caregivers on disease management and medications.
• Monitor patient progress and communicate changes to physicians and the interdisciplinary team.
• Collaborate with PT/OT/SLP, Medical Social Workers, and HH Aides to coordinate care.
• Ensure documentation is accurate, complete, and submitted within agency timeframes.
• Maintain compliance with Medicare Conditions of Participation and agency policies.
• Participate in case conferences and communicate with the clinical team.
• Travel throughout the assigned coverage area to provide patient care.
