Del Corazon Hospice in Santa Fe, NM is seeking a compassionate PRN Hospice Social Worker to join our team serving Santa Fe and surrounding Northern New Mexico communities. The role involves providing psychosocial assessments, emotional support, and care planning for patients and families.

Requirements include an MSW and an active New Mexico LMSW license, with hospice or healthcare experience preferred. A valid driver’s license and reliable transportation are also needed.

Qualifications

• MSW required; LMSW license in NM preferred.
• Hospice, home health, palliative care or healthcare experience preferred.

Responsibilities

• Provide initial social services assessments to hospice patients
• Provide emotional support and counseling to patients and families
• Assist with psychosocial assessments and care planning
• Support patients and caregivers with community resources and end-of-life planning
• Collaborate with the interdisciplinary hospice team
• Assist with crisis intervention, grief support, and advocacy
• Maintain timely and accurate documentation
• Participate in IDG meetings as needed
