Job summary

Ambercare Hospice in Alamogordo, NM, with occasional travel to Las Cruces, NM, seeks a compassionate PRN LMSW or LCSW to join our growing team. We offer a supportive, mission-driven environment with a focus on patient and family-centered hospice care.

The role requires NM Master’s Degree in Social Work, LMSW or LCSW licensure, and health care/hospice experience. Travel and a driver’s license are needed to coordinate visits and ensure quality care.

Qualifications

• New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work (LMSW or LCSW).
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patient and family/ caregivers and staff as needed in relation to stress and other identified coping difficulties.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs.
• Serves as liaison between patients and families/caregivers and community agencies
