Job summary

Del Corazon Hospice in Santa Fe, NM seeks a compassionate PRN Hospice Social Worker to support patients and families in Santa Fe and Northern New Mexico. Responsibilities include psychosocial assessments, counseling, and care planning, collaborating with the interdisciplinary team.

MSW is required with NM LMSW license; hospice experience preferred. Flexible PRN scheduling and a supportive leadership team accompany a competitive per-visit pay structure and mileage reimbursement.

Qualifications

• MSW required.
• Current New Mexico LMSW license required.
• Hospice, home health, palliative care, or healthcare experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Provide initial Social Services assessments to hospice patients
• Provide emotional support and counseling to patients and families
• Assist with psychosocial assessments and care planning
• Support patients and caregivers with community resources and end-of-life planning
• Collaborate with the interdisciplinary hospice team
• Assist with crisis intervention, grief support, and advocacy
• Maintain timely and accurate documentation
• Participate in IDG meetings as needed

We are seeking a compassionate and dependable PRN Hospice Social Worker to join our growing team serving Santa Fe and surrounding Northern New Mexico communities.
