Job summary

Ambercare Hospice in Las Vegas, NM is seeking a compassionate PRN Social Worker to join our refreshed team. You will perform psychosocial assessments, support patients and families, and coordinate resources to ensure compassionate, quality care.

Qualifications

• New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work (LMSW or LCSW) in New Mexico.

Responsibilities

• Performs psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures needs are addressed in the care plan.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patients and families/caregivers and staff on stress and coping difficulties.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Assists with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver and connects to community resources.
• Serves as liaison between patients/families and community agencies.

Education

MSW (Master’s Degree in Social Work)

LMSW or LCSW licensure
