HealthPro Heritage is seeking a Home Health Physical Therapist Assistant to support patients in Guilford, CT and surrounding areas. The PTA will assist therapists, implement treatment plans, and document patient progress in an EMR system.

The ideal candidate holds an Associate’s degree in Physical Therapist Assisting with active state licensure or eligibility and strong communication skills to educate patients and families. This role offers a flexible PRN schedule in a collaborative care setting.

Qualifications

• Associate degree required for PTA.
• Current state PTA licensure or license eligible.
• Strong assessment, manual therapy, and communication skills.

Responsabilités

• Assist in implementing treatment plans designed by Physical Therapists; provide direct patient care.
• Monitor patient progress and document outcomes; educate patients and families on treatments.
• Accurately document treatments, progress, and responses in the EMR system and facility policies.
• Collaborate with PTs, healthcare professionals, and staff for coordinated patient care.
