Job summary

Qualifications

Active CNA or HHA certification in Indiana
Previous home health or similar care experience preferred
Reliable transportation to patient homes in Bloomington/Nineveh area
Strong communication and patient-centered care
HIPAA compliance and confidentiality

Responsibilities

Provide personal care including bathing, grooming, dressing, and hygiene support
Assist patients with mobility, transfers, and ambulation
Monitor and document health status, report changes to supervising nurses
Support meals and nutritional needs
Perform light housekeeping and maintain a safe home environment
Offer companionship and emotional support to patients and families
Follow individualized care plans and home health protocols
