Job summary

Bishop Gadsden in Charleston, SC is seeking a licensed Physical Therapist to screen, evaluate, and revise care plans for residents under physician supervision. You will supervise aides, set treatment goals, and reassess progress every 30 days to ensure appropriate care.

Qualifications

• Must have a degree in Physical Therapy from an accredited program.
• Must meet all certification and licensure requirements as established by the state and national board associations.
• Active South Carolina license required.
• Active CPR certification.
• Comfortably uses word processing & spreadsheets for reporting; familiar with programs including Rehab Optima, Ultipro, Healthcare Source, MedBridge, PlanSource, Word, Excel, PowerPoint, RightTrack, etc.
• Ability to communicate clearly and effectively both verbally and written.
• Excellent analytical and thinking skills, and a team player.

Responsibilities

• Screen all residents on a quarterly basis to identify declines in functional abilities and determine if Physical Therapy intervention is indicated.
• Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
• Re-assess patients and caseload at least every 30 days to determine need for continued services or discontinuation of services based on progress; carry out treatment plan as indicated.
• Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and modify treatment plan as indicated.
• Provide thorough discharge planning to maximize independence in the least restrictive environment.
• Educate staff, family, and caregivers as indicated.
• Complete all required documentation according to guidelines specified in Documentation Manual.
• Maintain professionalism and follow all national/state association regulations.
