Flexible shifts include full-time, part-time or PRN options, with duties spanning assessment, treatment planning, and patient education.

Qualifications

• PT degree from an accredited Physical Therapy program.
• Current Physical Therapist license in the state employed.
• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Establishes and administers a treatment plan with specific therapy goals.
• Assesses patient needs and plans, evaluates and modifies care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals.
• Sets realistic and achievable goals for patients/residents.
• Documents and records patients’/residents’ condition and progress per regulations.
• Educates patients and families in appropriate therapy methods.
• Optimizes functionality by screening residents; directs and oversees processes.
• Communicates patient status to patients, families and professionals.
• Ensures punctuality and regular attendance for shifts.

Education

Bachelor’s, Master’s or Doctorate Degree from an accredited Physical Therapy program
