Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) to assess patients needing rehabilitation and to develop individualized treatment plans. You’ll provide direct care, document progress, and educate patients and families while working under the supervision of the Director of Rehabilitation.

Prerequisites include licensure and a degree in Physical Therapy; experience in long-term or geriatric care is a plus.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid professional license and registration in state.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills and timely charting.
• Perform evaluations, progress notes, recertification and discharge docs.
• Collaborate with all disciplines for patient-centered care.
• Demonstrate exceptional patient service and professionalism.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment programs with specific goals.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and coordinate with disciplines.
• Maintain HIPAA compliance and confidentiality.
