OneWell Health Care seeks a dedicated Speech-Language Pathologist (SLP) for per-diem, home health in Pennsylvania. Assess and treat IDD individuals in home and community settings; flexible schedule and part-time coverage. Requires CCC-SLP, PA licensure, and willingness to travel.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and valid SLP license in the state of practice.
• CCC-SLP Certification (ASHA).
• Strong clinical, communication, documentation, and organizational skills.
• Valid driver’s license and willingness to travel to home/community-based service locations.
• Experience with IDD, home/community-based services, and AAC/dysphagia preferred.
• English proficiency required; bilingual (Spanish) a plus.

Responsabilités

• Conduct speech, language, cognitive-communication, AAC, and swallowing evaluations.
• Develop and update individualized treatment plans.
• Provide evidence-based speech, communication, AAC, feeding, and swallowing therapy.
• Collaborate with families, caregivers, and interdisciplinary team members.
• Educate caregivers on communication strategies, AAC use, and safe swallowing practices.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
