Qualifications

• Minimum of one year of clinical experience.

Responsibilities

• Provide skilled physical therapy services in patients' homes under the supervision of a Physical Therapist.
• Perform transfer training and mobility instruction.
• Educate patients and caregivers on exercises and safety techniques.
• Monitor patient progress and report changes to supervising Physical Therapist.
• Collaborate with care team and complete electronic documentation.
• Maintain accurate and timely documentation.
