OneWell Health Care seeks a dedicated Per Diem Physical Therapist to provide home and community-based care in Pennsylvania. You’ll evaluate mobility, develop goals, and collaborate with families and professionals to improve independence.

Requirements include a PT degree (DPT or MSc), active PA PT license, CPR, and willingness to travel. Flexible scheduling and field-based work support diverse communities throughout PA.

Qualifications

• Doctorate or Master's degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice.
• Current CPR certification and driving ability.
• Experience in home health or IDD care is preferred.

Responsabilités

• Evaluate mobility, strength, gait, balance, and safety needs in home/community settings.
• Develop and update individualized, goal-based treatment plans.
• Provide evidence-based PT services to improve independence and function.
• Educate patients and caregivers on mobility techniques and safety.
• Recommend mobility aids and adaptive equipment as needed.
• Document clinical findings accurately in the EHR system.
• Collaborate with caregivers and interdisciplinary team members.
• Participate in required meetings and trainings.
• Travel to home and community-based service locations as required.
