Job summary

OneWell Health Care is seeking a dedicated Per Diem Physical Therapist in Phoenixville, PA to provide personalized therapy in home and community settings. You will evaluate mobility, develop goal-based plans, and collaborate with caregivers to improve independence.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of practice (PA).
• Current CPR certification (preferred or required per policy).
• Strong clinical, documentation, organizational, and communication skills.

Responsibilities

• Evaluate mobility, strength, balance, gait, posture, and safety needs.
• Develop and update individualized treatment plans.
• Provide evidence-based physical therapy in home and community-based settings.
• Help individuals improve mobility, coordination, physical function, and independence.
• Recommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.
• Educate individuals, families, and caregivers on safe mobility techniques.
• Collaborate with caregivers and interdisciplinary team members.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.
