Job summary

Associated Home Care is seeking a per diem Registered Nurse to administer and monitor patient care per the Care Plan. The role ensures compliance with federal, state, and local regulations and company policies while delivering skilled nursing services.

The RN will collaborate with the Director of Nursing, caregivers, physicians, and care team members. Strong communication, problem-solving, and computer literacy are essential for high-quality patient care and coordination.

Qualifications

• RN licensed in practicing state
• One+ year experience in clinical home health services preferred
• Licensed driver with reliable transportation and insurance

Responsibilities

• Follow Care Plan with Nursing Director and care team
• Conduct on-site orientation and supervise home health aides
• Provide ongoing instruction and reinforce safety/precautions
• Review care plans from ASAP and orient aides accordingly
• Document services and client condition changes
• Communicate concerns to ASAP Case Managers or family
• Perform other duties as assigned
