Qualifications

• Minimum one year of clinical experience, preferably in home health.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Provide skilled occupational therapy services in patients' homes.
• Perform initial evaluations, reassessments, and fall risk assessments.
• Develop and implement individualized treatment plans.
