Job summary

Always Best Care Senior Services is seeking a Home Health Occupational Therapist (OT) and Certified Occupational Therapy Assistant (COTA) in Honolulu, Hawaii. This per diem position allows you to build your own caseload while delivering vital home health services to seniors.

Qualifications

• Licensed Occupational Therapist or Certified Occupational Therapy Assistant in Hawaii.
• Minimum one year of clinical experience, preferably in home health.
• Strong communication and documentation skills required.

Responsibilities

• Develop individualized treatment plans and conduct evaluations.

Home Health Occupational Therapist (OT) & Certified Occupational Therapy Assistant (COTA) - Per Diem / PRN

OT Pay: $80 per visit or $90,000-$110,000 annually (based on caseload)

COTA Pay: $60 per visit

Location: O‘ahu, HI (Reliable transportation required)

Schedule: Flexible / Per Diem (PRN) - Build Your Own Caseload

What You'll Do

• Provide skilled occupational therapy services in patients' homes
• Perform initial evaluations, reassessments, and fall risk assessments (OT)
• Develop and implement individualized treatment plans
• Improve independence with activities of daily living (ADLs), mobility, safety, and home management tasks
• Educate patients and caregivers on adaptive techniques and fall prevention strategies
• Collaborate with nurses, therapists, physicians, and caregivers
• Maintain accurate documentation in accordance with Medicare home health standards

What We're Looking For

• Current Hawaii license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)
• Minimum one year of clinical experience (home health experience preferred)
• Strong communication, organizational, and documentation skills
• Ability to work independently in the field
• Valid driver's license and reliable transportation
• Passion for serving seniors, kupuna, and local families
