Job summary

Always Best Care Home Health in Oahu, Hawaii is hiring Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) for in-home care. Per diem schedule with flexible caseload building and the chance to deliver one-on-one therapy in patients' homes.

You'\''ll conduct evaluations, develop treatment plans, and collaborate with an interdisciplinary team while maintaining thorough documentation and ensuring patient independence.

Qualifications

• Active Hawaii OT license required.
• Active Hawaii COTA license required.
• Minimum one year of clinical experience; home health experience preferred.

Responsibilities

• Deliver occupational therapy services in patients' homes.
• Complete initial evaluations and follow-up visits.
• Perform fall risk assessments and home safety evaluations.
• Develop and implement individualized treatment plans (OT).
• Assist with treatment interventions established by the supervising OT (COTA).
• Educate patients and caregivers on adaptive techniques and equipment.
• Monitor patient progress and communicate changes with the interdisciplinary team.
• Complete accurate and timely clinical documentation.
