Job summary

Always Best Care Home Health is seeking compassionate Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide in-home therapy on O'ahu, Hawaii. Flexible per diem schedules allow you to build your own caseload while delivering one-on-one care in patients’ homes.

You’ll promote independence, conduct evaluations, and develop individualized plans in a supportive clinical team setting.

Qualifications

• Active Hawaii Occupational Therapist license.
• Minimum one year of clinical experience.
• Home health experience preferred but not required.
• Active Hawaii Certified Occupational Therapy Assistant license.
• Minimum one year of clinical experience preferred.

Responsibilities

• Deliver occupational therapy services in patients' homes.
• Complete initial evaluations and follow-up visits.
• Perform fall risk assessments and home safety evaluations.
• Develop and implement individualized treatment plans (OT).
• Assist with treatment interventions established by the supervising OT (COTA).
• Educate patients and caregivers on adaptive techniques and equipment.
• Monitor patient progress and communicate changes with the interdisciplinary team.
• Complete accurate and timely clinical documentation.
