Job summary

Roland Park Place, Inc. in Baltimore, MD, is seeking a part-time Social Worker (Exempt) for our 16-bed Skilled Nursing Facility.

The 0.6 FTE role focuses on providing psychosocial support to residents and families, coordinating care, and supporting discharge planning. The ideal candidate is an LCSW or LBSW licensed in Maryland with at least two years in healthcare, excellent communication, and a resident-centered approach in a collaborative interdisciplinary team.

Qualifications

• Licensed Clinical Social Worker (LCSW) or Licensed Bachelor Social Worker (LBSW) in Maryland.
• Minimum of two years of healthcare, skilled nursing, rehabilitation, or long-term care experience.
• Experience with discharge planning, care coordination, and interdisciplinary team collaboration.
• Knowledge of Medicare, MDS documentation, and long-term care regulations preferred.
• Excellent communication, conflict resolution, and organizational skills.
• Compassionate, resident-centered approach with the ability to build trusting relationships.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents.
• Advocate for residents' emotional well-being, dignity, and rights.
• Coordinate interdisciplinary care plans and participate in care conferences.
• Provide counseling, support, and crisis intervention to residents and families.
• Assist with discharge planning and coordinating post-discharge services.
• Connect residents and families with community resources and programs.
• Facilitate family meetings and resolve concerns and conflicts.
• Collaborate with physicians, nursing staff, therapy, admissions, and partners.
• Complete MDS documentation and maintain medical records per regulations.
• Coordinate admissions and serve as liaison with discharge planners.
• Support regulatory surveys and HIPAA compliance.
