Job summary

E & S Home Care Solutions, LLC is seeking a compassionate Registered Nurse (RN) Field Nurse to perform in-home assessments and manage care plans in South Plainfield, NJ. This field-based role offers flexible scheduling and meaningful, independent work delivering high-quality home health services.

The RN will conduct assessments, develop and update care plans, supervise HHAs, and ensure regulatory compliance while communicating clearly with clients and families.

Qualifications

• Active and unrestricted RN license in New Jersey.
• Home health experience preferred.
• Strong nursing assessment and documentation skills.
• Excellent communication and organizational skills.
• Ability to work independently and manage field visits.
• Valid driver's license and reliable transportation.
• Compassionate, dependable, and professional.

Responsibilities

• Conduct initial nursing assessments and reassessments in clients' homes.
• Develop, review, and update individualized plans of care.
• Perform supervisory visits for Home Health Aides (HHAs).
• Evaluate clients' health conditions and identify changes in care needs.
• Ensure compliance with New Jersey home care regulations and agency policies.
• Complete all required nursing documentation accurately and within established deadlines.
• Communicate effectively with clients, families, caregivers, and office staff.
• Provide guidance and education to HHAs regarding client care needs.
