Qualifications

Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
Active Tennessee Physical Therapist license.
Current CPR certification.
Valid driver's license and reliable transportation.
Strong clinical assessment and communication skills.
Ability to work independently and manage time effectively.
Home health experience preferred, but not required.

Responsibilities

Perform in-home evaluations to assess patient needs and develop individualized treatment plans.
Provide skilled physical therapy services to improve mobility, strength, balance, and overall function.
Monitor patient progress and modify treatment plans as appropriate.
Educate patients and caregivers on home exercise programs, safety measures, and injury prevention.
Complete accurate and timely clinical documentation per company policies and state regulations.
Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care.
Travel to patient homes within the assigned service area.
