Qualifications

Responsibilities

• Perform in-home evaluations to assess patient needs and develop individualized treatment plans.
• Provide skilled physical therapy services to improve mobility, strength, balance, and overall function.
• Monitor patient progress and modify treatment plans as appropriate.
• Educate patients and caregivers on home exercise programs, safety measures, and injury prevention.
• Complete accurate and timely clinical documentation in accordance with company policies and state regulations.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care.
• Travel to patient homes within the assigned service area.
