Job summary

AccentCare in Chattanooga, TN is seeking a Physical Therapist for Home Health. This part-time/PRN role offers flexible scheduling (M-F 8am-5pm) and a caseload in our Home Health program.

You will evaluate and treat patients, document care, collaborate with an interdisciplinary team, educate families, and travel to patient locations. Requirements include CAPTE-approved master's, at least nine months' PT experience, Tennessee license, CPR, and driving.

Qualifications

• Master's degree in physical therapy approved by CAPTE.
• Minimum nine months of experience as a physical therapist.
• Licensed to practice in the state of operation.
• Valid CPR certification and a valid driver’s license with ability to travel.

Responsibilities

• Provides patient care and documents treatment per orders and policy.
• Performs patient evaluations per policy and procedure; selects diagnostic material.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with objectives and priorities.
• Educates and counsels patients and presents information to physicians and team.
• Meets productivity standards and uses work hours efficiently.
