Job summary

HealthBar in Pleasant Prairie, WI seeks a Nurse Practitioner (NP or CNS) for a flexible 20 hours/week, benefits-eligible role starting at $57/hour.

You will work at an employer-sponsored clinic serving Kem Krest employees, delivering comprehensive primary care in a prevention-focused model outside traditional insurance. Enjoy autonomy with an experienced clinical team and meaningful patient relationships.

Qualifications

• Master's degree in Nursing from an accredited program.
• Active, unrestricted Wisconsin RN license.
• Active, unrestricted FNP certification (AANP or ANCC).
• Active Wisconsin NP Specialty Certification.
• BLS certification (AHA or Red Cross) within 90 days of hire.
• Must be authorized to work in the U.S. without employer sponsorship.

Responsibilities

• Educate patients on prevention, self-management, and chronic conditions.
• Provide proactive, relationship-based primary and occupational care.
• Coordinate follow-up care and care management outreach.
• Counsel on medications, lifestyle changes, and interactions.
• Order and interpret labs, imaging, and point-of-care tests.
• Develop individualized treatment plans considering social determinants of health.
• Treat acute illnesses and chronic conditions across in-person, virtual, and employer settings.
• Collaborate with specialists, behavioral health providers, and employer teams to optimize outcomes.
• Support HealthBar wellness initiatives and preventive programs.
