You will perform routine rounds, manage chronic diseases, and collaborate with interdisciplinary teams to uphold high-quality, person-centered care.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills and compassionate communication.
• Strong collaboration across interdisciplinary teams.

Responsibilities

• Provide comprehensive primary care services within skilled nursing and senior living communities.
• Routine rounding, proactive chronic disease management, and care coordination with facility partners.
• Collaborate with physicians, behavioral health specialists, and care coordinators to create integrated care plans.
