Theoria Medical is seeking a full-time Advanced Practice Provider to deliver high-quality care for residents in local skilled nursing facilities. You will perform daily rounds, manage treatment plans, and collaborate with a clinical team to ensure continuity of care.

Qualifications

• Active U.S. medical license and board certification in relevant AP roles.

Responsibilities

• Perform daily rounds and manage treatment plans.
• Address acute/emergent issues and coordinate with facility staff.
