Theoria Medical seeks a part-time Advanced Practice Provider to serve local skilled nursing facilities within a mission-driven care community. You will care for residents over time, building trust and delivering exceptional care with flexibility and autonomy, supported by intuitive technology and a collaborative team.

Qualifications

• Active U.S. license and board certification as NP or PA (per listed specialties).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Perform daily rounding, prescribe medications and treatment plans, address acute/emergent issues.
• Communicate with facility nursing staff via secure messaging or phone.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and facility policies.

Education

Active U.S. license

Board-certified by ANCC/AANPCB/PNCB or AACN (NPs) or NCCPA (PAs)
