Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living assistance and personal care in patients' homes according to an established care plan.

Responsibilities include assisting with daily activities, taking vital signs when ordered, safe transfers, and documenting changes for the supervising nurse. Flexible, part‑time hours and a competitive salary are offered, with emphasis on safety and patient comfort.

Qualifications

• Meets training requirements per state and federal laws.
• Ability to read, follow written instructions and document care given.
• Current CPR certification per state regulation.
• PPD or chest X-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation in Maxim competency program.

Responsibilities

• Assist patients with daily living activities per plan of care (baths, hygiene, dressing, feeding).
• Take and record vital signs when ordered.
• Assist with safe transfers and ambulation.
• Carry out exercises and routine therapy as ordered.
• Perform range-of-motion and simple procedures as directed.
• Handle patient laundry and safety equipment.
• Promote home safety and observe for emergencies.
• Communicate clearly with client, family, and office; report changes to nurse supervisor.
