Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes with daily living activities and personal care according to an established care plan.

You will follow the plan of care, communicate effectively with clients and caregivers, and report patient status to the supervising nurse. The role offers flexible hours and a competitive salary in a part-time capacity.

Qualifications

• Meets all training requirements per state and federal laws.
• Ability to read, follow written instructions and document care delivered.
• CPR certification maintained as required by state regulations.
• PPD or chest X-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation in Maxim’s competency program.

Responsibilities

• Assist patients with activities of daily living per plan of care.
• Perform baths, oral hygiene, dressing, toileting, and feeding as instructed.
• Take and record vital signs when ordered.
• Ensure safe transfers and ambulation as directed.
• Assist exercises and ROM as ordered.
• Care for patient laundry and household safety.
• Document patient condition and report changes to nurse supervisor.
