Job summary

• Reads and follows written instructions and documents care
• Ability to communicate clearly with client and family
• Maintains CPR certification as required

Responsibilities

• Assist patients with baths, oral care, dressing, toileting, and feeding
• Take and record vital signs as ordered and monitor patient conditions
• Safe transfer and ambulation; assist with range of motion and exercises
• Observe, document, and report changes in patient condition
• Follow the plan of care and complete assigned tasks
• Meet safety needs and observe home safety practices
