Job summary

Pinnacle Home Care is hiring an Occupational Therapist Assistant (OTA) on a PRN basis to support at-home patient care across Florida. The OTA collaborates with the Occupational Therapist to implement and adjust the rehabilitative plan, document progress, and educate patients and families on exercises and self-care.

The role emphasizes thorough documentation, patient safety, and teamwork in a high-quality home health environment that has earned recognition for patient-centered care.

Qualifications

• Graduate of an accredited occupational therapy assistant program.
• Current certification or license as required by state law.
• Current BLS Certification.
• 6–12 months Home Health experience preferred.
• Demonstrates good verbal and written communication and organization skills.
• Licensed driver with a vehicle in good working order.

Responsibilities

• Provide client care services delegated by the Occupational Therapist.
• Assist in evaluation and development of the rehabilitative plan of care and periodic re-evaluation.
• Prepare and maintain daily progress notes and periodic summaries.
• Instruct client and family in exercises and self-care.
• Rapport with client, family, and health team to support therapeutic goals.
• Record observations and report changes to OT and physician.
