Job summary

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, seeks an Occupational Therapy Assistant (OTA) to join our award-winning team providing in-home patient care across Clermont, Haines City, Davenport and surrounding areas.

You will work under the OT direction, contribute to care plans, document progress, and educate patients and families to promote independence and safety in daily living.

Qualifications

• Graduate of an accredited occupational therapy assistant program.
• Current certification or license as required by state law.
• Current BLS Certification.
• 6–12 months Home Health experience preferred.
• Strong verbal and written communication and organization skills.
• Licensed driver with reliable vehicle.

Responsibilities

• Provide client care under the OT's direction.
• Assist in evaluating and developing the plan of care and re-evaluation as required.
• Prepare daily progress notes and periodic reports.
• Instruct clients and families in exercises and self-care.
• Document findings in the clinical record and report changes to OT.
• Assist in discharge planning and coordination of home visits.
