Job summary

Hueman is actively hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Madison, GA. This role lets you shape a flexible schedule while making meaningful connections with patients outside the clinic.

Nurse Practitioners must hold an active NP license, national board certification, and a Master’s degree in nursing. Travel to patients’ homes is required, with liability insurance and dedicated scheduling support provided.

Qualifications

• Master’s degree in Nursing (NP) or commensurate experience with NP license.
• Active State NP license.
• National board certification as NP.
• BLS certification in good standing.

Responsibilities

• Travel to and from patients’ homes to perform HRAs.
• Conduct comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation for patient safety and care coordination.
• Provide basic education on wellness and preventative healthcare.
