Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Pumpkin Center, CA. This role offers flexible scheduling and the opportunity to work outside a traditional clinic setting, traveling to patients’ homes to perform non-invasive assessments.

Qualified NPs will hold an active state license, national board certification, and a current BLS; 12 months of paid advanced clinical provider experience is

Qualifications

• Master’s Degree OR commensurate experience and NP license.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
