Qualifications

• Master's degree in nursing with NP license or commensurate experience.
• Active State NP license and national board certification required.
• Current BLS certification.

Responsibilities

• Travel to patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document accurately.
• Conduct medical reconciliation to ensure safety and compliance.
• Provide basic education on wellness and prevention.
