Job summary

Hueman in Marion, Kentucky seeks Nurse Practitioners to perform in-home Health Risk Assessments for Medicare and Medicaid members. Travel to patients’ homes is required; HRAs involve comprehensive, non-invasive assessments and documentation, with no prescribing in home.

Ideal candidates hold a Master’s Degree or equivalent experience, active State NP license, board certification, and current BLS. A flexible schedule, training, and support are offered to help integrate into the local healthcare

Qualifications

• Master’s Degree or commensurate experience and NP license.
• Active State NP license.
• Active National board certification.
• BLS certification required or current.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative healthcare.
